Basic Life and AD&D

Basic Life Insurance pays your beneficiary a lump sum if you die. AD&D provides another layer of benefits to either you or your beneficiary if you suffer from loss of a limb, speech, sight, or hearing, or if you die in an accident. The cost of coverage is paid in full by Pentagon Technologies. Coverage is provided by Lincoln Financial Group.

Basic Life insurance – You receive Basic Life insurance coverage at no cost to you, and you do not need to enroll in any other health and protection program. Basic Life insures you for 100% of annual salary, up to a maximum of $300,000.

Imputed Income: Under current tax laws, imputed income is the value of your Basic Life insurance that exceeds $50,000 and is subject to federal income, Social Security and state income taxes, if applicable. This imputed income amount will be included in your paycheck and shown on your W-2 statement.

Basic AD&D is included with your Basic Life coverage and provides you specified benefits for a covered accidental bodily injury that directly causes dismemberment (i.e., the loss of a hand, foot or eye). If death occurs from an accident, 100% of the AD&D benefit would be payable to your beneficiary(ies).

    Voluntary Life and AD&D

    Voluntary life and AD&D insurance allows you to purchase additional life insurance to protect your family’s financial security. Coverage is available for your spouse and/or children if you purchase coverage for yourself.

    Evidence of Insurability (EOI)

    If you choose an amount that exceeds the guaranteed issue for Voluntary Life and AD&D, you will need to complete the Evidence of Insurability (EOI). Please follow the instructions below to submit your information online.

    1. Log in to LincolnFinancial.com. First time user? Register using company code: PENTAGONT.
    2. Your information may pre-populate on Lincoln’s portal (LincolnFinancial.com). Review it and ensure that the portal is displaying the correct coverage type you’re applying for.
    3. Select Complete Evidence of Insurability
    4. Answer questions about you (and family members, if applicable).
    5. Review your responses, then electronically sign and submit your application. Save your confirmation.

    Employee Amount: Minimum of $10,000 increments up to a lesser of 5x annual salary or $500,000.

    Guarantee Issue for newly eligible employees: $200,000

    Spouse Amount: Minimum of $5,000 increments up to $100,000. Limited to 50% of employee amount.

    Guarantee Issue: $30,000

    Child(ren) Amount: Minimum of $250 no more than $10,000.

    Guarantee Issue: Full Amount
